Clinical Manifestations Gastrointestinal bleeding is a common first manifestation of peptic ulcer disease in the older patient. As a result, these patients may come to medical attention because of systemic manifestation of subacute or chronic blood loss such as shortness of breath, chest pain or loss of consciousness. Thirty-five per cent to 45% of all cases of active upper gastrointestinal hemorrhage occur in patients older than 60 years of age; half of these cases are caused by peptic ulcer disease. Other causes of upper gastrointestinal hemorrhage in elderly patients are shown in Table 4. Bleeding from any cause in an elderly patient, however, may be aggravated by the concomitant use of anticoagulant drugs (such as coumadin or heparin) or antiplatelet agents (such as ticlopidine, and ASA and other NSAIDs).
